Playing a mainstream variant of Standard American, you hold:

Partner opens 1 Heart. What do you bid? What do you rebid
if partner then makes a minimum rebid in hearts or NT?

This is not from a problem hand -- it's a construction
to help me understand standard bidding. So feel free to
explain how minor changes to the hand (e.g. spades slightly
better or slightly worse) would affect your bidding.

I respond 1S. Even if we're playing strong jump shifts (which many
people these days aren't), a strong jump shift should definitely play
either in the jump shift suit, or opener's suit, or notrump. This hand
can play in diamonds as well.

After 1H-1S; 2H, I'll rebid 3D, which is forcing to game.

After 1H-1S; 1N, it depends on what system we're playing. In my
preferred methods, 3D now is forcing with slam interest.

prolly 3D in both cases
I do not know if SA play either checkback or NMF, but if so, then 3D may
have a different meaning , Like a gametry with 5-5

I would respond 1S, then 3D over most rebids by opener. This will be game-forcing in all cases:
1H-1S; 1NT-3D, game-forcing jump shift
1H-1S; 2C-3D, same
1H-1S; 2D-4D, a slam try, not a game invitation, but make sure partner is on the same page!
1H-1S; 2H-3D, new suit at the three level by an unlimited hand, game forcing.
1H-1S; 2S-3D, ostensibly a game try, will follow with 4NT. The 3D call may help partner bid 7.

The first and fourth sequences, while game forcing, don't really do justice to the hand; I'd like to make an immediate jump shift to signal slam interest but the spades are fairly weak and American-style strong jump shifts have always prohibited having a side suit. Still, you might compare whatever happens with a sequence like 1H-2S; 2NT-3D; something-4D.

1S would be a normal action. You need to have agreements to bid this hand easily, because some delayed heart support call may be misleading. Personally, I play that a simple new suit denies 3 card heart support, so later auctions are easier.

How do you escape from the major into the suit(s) you can make slam in?

Playing a natural canapé system, I'd bid as most everyone has suggested. Reply 1S then bid Diamonds at the 3 level. Game forcing; doesn't preclude playing in Hearts.
